Depression

Depressive disorders are characterized by persistent low moods, loss of interest and reduced energy. They often impair day to day functioning.

A major depressive disorder is characterized by one or more depressive episodes (i.e. at least two weeks of depressed mood or loss of interest accompanied by at least four additional symptoms of depression).

Causes

  • There is a genetic predisposition to depression, especially when of early onset. The number and identity of the genes are largely unknown but the serotonin transporter gene is a candidate.
  • Adversity and emotional deprivation early in life also predispose to depression.
  • Depressive episodes are often, but not always, triggered by stressful life events (especially those that involve loss), including medical illnesses.
  • Certain personality types are more prone to depression than others. People with low self esteem, who are easily overwhelmed by stress or who lose hope easily are likely to experience depression.
  • Environmental influences are major contributory factors in the development of depression such as continuous exposure to neglect, abuse, violence, humiliation etc.
  • Associated biological factors include variations in certain chemicals in the brain that lead to symptoms of depression.
  • Underlying medical conditions such thyroid disorders, vitamin deficiencies etc also are the causes

Symptoms

Symptoms may be mild, moderate or severe, may be episodic, recurrent or chronic.

Psychological symptoms

  • Depressed mood is the characteristic feature. There may be diurnal variation in mood
  • Loss of pleasure in life
  • Loss of interest in oneself and others
  • Low self esteem and ideas of hopelessness and worthlessness
  • Self blame and feelings of excessive or inappropriate guilt
  • Psychotic features (e.g. hallucinations, depressions, delusions, etc)
  • Suicidal thoughts

Somatic symptoms

  • Sleep disturbances such as initial insomnia, early morning wakening or hypersomnia
  • Fatigue, headache and various other pains
  • Anorexia, weight loss or weight gain and bowel disturbance
  • Poor concentration, psychomotor retardation and reduced sexual drive

Diagnosis

  • Depressed mood most of the day, nearly every day
  • Markedly diminished interest/pleasure in all or almost all activities
  • Significant weight loss or weight gain. Variable appetite
  • Loss of energy or fatigue nearly every day
  • Loss of confidence or self esteem
  • Recurrent thoughts of suicide or death or any suicidal behavior
  • Diminished ability to think or concentrate
  • Unreasonable feelings of self reproach or excessive or inappropriate guilt
  • Psychomotor agitation or retardation
  • Insomnia or hypersomnia
  • Change in appetite

It is important to consult a physician for diagnosis. Physical examination and other investigation to determine underlying medical condition are important.
